Student Success Plan

The Student Success Plan is a family-friendly tool developed by the Nevada Department of Education, Office of Early Learning and Development to help you share your child’s unique strengths and needs with their school team. This simple template supports clear communication during important transitions—such as starting school, moving between grade levels, or adjusting to a new learning environment.

Designed to complement, not replace, formal supports, the Student Success Plan gives families a voice in helping educators better understand their child.

What the Student Success Plan Offers

Why It Matters

Important Note

The Student Success Plan is an informal tool and does not replace a formal Individualized Education Program (IEP) or Section 504 Plan. Instead, it works alongside these supports to provide additional insight and collaboration.
